Do I need a referral?

No! Connecticut is a direct access state, which mean's you don't need a referral from a physican to come see us. You can book your first appointment directly with us.

How do I book?

  1. Book a free 15 minute phone consultation online
  2. Call/text: 860-418-7643

What should I bring?

  • Comfortable workout clothes
  • Athletic shoes
  • Any medical records or imaging (if you have them)

What happens at my first visit?

The number one priority at our initial evaluation is to help you make sense of your pain. We'll drill down to the root cause by watching you move, taking a careful history, and getting started on a trial treatment. We'll answer your questions, discuss your goals, and and build out a treatment to get you there. You'll leave with total clarity on what you need to do to overcome your pain and get back to doing the things you love.

How long are sessions?

Sessions are typically 60 minutes for our evals and follow-ups. We do offer 30 minute sessions as well when appropriate.

Where are you located?

INSURANCE AND PAYMENT

Do you take insurance?

We are considered an out-of-network clinic, which means we don't bill insurance directly.

Why? It allows us to:

  • Spend a full hour with you (not 15 minutes)
  • See one patient at a time (not 3-4 simultaneously)
  • Create personalized plans (not cookie-cutter protocols)
  • Focus you and your goals

How much does it cost?

  • Initial Evaluation: $200

After your evaluation, Dr. Matt will be able to give you an estimate of how many sessions you will need for your plan of care. Every patient is different, and no two injuries present the exact same. He'll make sure you know exactly what to expect going forwards cost-wise. Most patients utilize one of our plan-of-care packages, which Dr. Matt will go over during your first visit.

Can I use my HSA or FSA?

Yes! You can link your HSA or FSA card directly in your patient portal. We also provide all the documentation you need to seek reimbursement through your out-of-network benefits.

Will my insurance reimburse me?

Every insurance plan is different. You'll need to verify your out-of-network benefits to determine exactly what their reimbursement policy is.

How it works:

  1. Payment is due after each visit.
  2. We provide you with a superbill with the proper medical codes
  3. You submit it to your insurance directly or through our portal
  4. They reimburse you based on your plan

We provide all documentation you need as well as a portal to verify your benefits and submit your superbills. You can also call your insurance beforehand to ask about out-of-network PT benefits.

What payment methods do you accept?

We accept cash, credit/debit cards, HSA/FSA card, or venmo. Payment due at time of service.

TREATMENT

Can I keep training during PT?

Yes! During your eval, we'll come up with a plan to keep you moving throughout your treatment.

Complete rest is rarely the answer. Our bodies were designed to heal through movement. Your training might look different for a period of time, but we provide all the support you need along the way so you know exactly what to do, and what not to do, to support your recovery.

What makes Pack PT different from other physical therapy clinics?

Pack Performance Physical Therapy is part of the next generation of performance based physiotherapy clinics around the country.

  • We are a fitness forward and will challenge you appropriately throughout rehab.
  • We focus on getting to the root cause of your pain so you have 100% clarity about what you are feeling. We don't just treat symptoms!
  • Every session is fully one-on-one, no juggling multiple patients and dividing our attention.
  • We priortize fundamentals and consistency. Do the basics really well and focus on getting 1% better each day to create sustainable habits leads to long term success!

At Pack, we treat the whole person, not just the diagnosis. You aren't a shoulder or a knee to use, you're a human with real goals and we're ready to help you reach them!

Do you do manual therapy?

Yes! Manual therapy can be a helpful component of your treatment plan. Manual therapy helps reduce pain, improve range of motion, and improve muscle firing patterns. Here's some of the ways we use it:

  • Joint mobilizations to improve range of motion
  • Joint manipulations to reduce pain and muscle guarding
  • Dry needling to reduce pain and improve range of motion

Manual therapy by itself won't solve your problem, that's why we always pair exercise with any manual therapy technique to lock in the changes and create lasting improvement.

What is dry needling and do you offer it?

Dry needling is a manual theray that involves inserting fine, filiform needles into skeletal muscle to treat trigger points, as well as connective tissue such as tendons and scar tissue. Dry needling helps relax trigger points to reduce pain and improve range of motion.

Dr. Matt is certified in trigger point dry needling through Myopain Seminars. He'll use dry needling when appropriate to help reduce pain and improve range of motion.

Manual therapy alone won't solve your pain, that's why we always reinforce the dry needling with proper mobility, strength and motor control drills afterwards. That's how we make the results stick.

Dry needling opens the door, you need to step through it.

How is dry needling different than acupuncture?

Dry needling and acupuncture utilize similar needles but apply them differently. Acupuncture is based on eastern medicine principles and typically invovles widespread needle application on a superficial level.

Dry needling tends to be more targeted to the specific muscle dysfunction you are experiencing and the needle is inserted deeper into the muscle belly to stimulate trigger points. We identify the proper muscles to needle based on our movement exam and pain history. Dr. Matt is trained to find the trigger points that are pain generators for you and treat them safely.

What are the side effects of dry needling?

The primary side effect of dry needling are soreness in the muscles that have been treated and occasionally bruising at the site where the needle was inserted. You may feel familiar pain referrals (pain in other areas of your body that are caused by local trigger points). Soreness typically goes away in 24-48 hours, and Dr. Matt will make sure you go home with the right exercises to support the dry needling treatment.

What conditions respond well to dry needling?

Myofascial pain conditions respond well to dry needling, including acute muscle injuries (strains), overuse injuries, tenidonpathies, referred pain (just to name a few!). There's also emerging evidence around the effectiveness of dry needling and chronic pain conditions such as fibromyalgia.

What's your treatment philosophy?

Our approach is built on a three step process:

1. Know your pain: Our thorough movement exam, combined with your pain experience, will identify the root cause of your pain and movement dysfunctions.

2. Create a plan: You’ll receive a customized plan based on your individual needs.

3. Build resilience for life: We don’t stop at improving your pain. We help you develop the tools and systems to build a resilient body for years to come!

LOGISTICS

How many sessions will I need?

That depends entirely on your injury/pain presentation. Most patients require between 6 and 10 visits. Acute or recent injuries typically require fewer sessions, while chronic pain presentations require a longer commitment.

Most clients see significant improvement within 2-3 weeks.

We'll give you a realistic estimate after your initial evaluation.

How often do I come in?

Typically 1-2 times per week depending on your condition and goals.

  • Early on: 1-2x/week to build momentum
  • As you improve: Once per week or every other week
  • Near the end: Once every 2-3 weeks for check-ins

We're flexible with your schedule.

What's your cancellation policy?

We require 24 hours notice for cancellations or rescheduling.

Late cancellations (less than 24 hours) or no-shows are charged 50% of the session fee.

Life happens—if you have an emergency, reach out and we'll work with you. You're allowed one "oops" during your plan of care with us where we'll forgive a late cancellation at the discretion of our team.

Do you offer virtual/telehealth sessions?

Yes! Virtual sessions via your Jane App for follow-ups, exercise review, and out-of-town clients.

Initial evaluations are best in person, but we can discuss virtual options case-by-case.

STRENGTH AND CONDITIONING

Do I need to finish PT first before starting S&C?

If you're currently working on a plan of care with Dr. Matt, we recommend you finish that out before transitioning over to S&C. Very often, the end of a PT plan feels like a strength and conditioning session since the way we approach rehab at the point is to build your strength and work capacity. Making the transition with Dr. Matt will be seamless and you honestly won't even notice a difference!

What if I've never lifted weights before?

Everyone starts their fitness journey somewhere. If you've never lifted weights before, Dr. Matt will put you through a thorough assessment to guage your movement patterning, load tolerance, work capacity to determine the right starting point for you.

How is this different from a CrossFit gym or regular gym?

Typical CrossFit gym programming is designed for groups and is meant to be broad, general, and inclusive. It works for most people, most of the time. The one-on-one work you'll do with Dr. Matt is tailored to your ability level and injury history. We put an emphasis on the mastering the fundamentals while also making it meaningful to your life and fitness goals.

How many days per week do I need to commit?

It depends on how comfortable training on your own while also making sure you stay consistent. Three days per week of intentional exercise is our general recommendation for most people. Of course that answer varies with what other activities you are doing and what your goals are.

For someone totally new to working out, strength training twice a week would be a great place to start while filling in some time outside the gym with other activities like biking, running, walking, hiking, pickelball, etc.

For someone looking to enhance their performance or supplement a current training program, once per week is the sweet spot.

How much does it cost?

Sessions are $150 for 60 minutes. Ask about our packages to bundle sessions and save on the per-session cost!

Is this separate from PT or part of it?

Strength and conditioning is a separate service we offer, but goes hand-in-hand with PT. It's a great way to transition out of PT into a long term fitness regimen to keep you strong for a lifetime and stop injuries from coming back. It's also your solution to improving performance in or out of the gym, setting a PR in a half-marathon, or providing long term accountability.

What equipment do I need?

Just bring yourself and a pair of good training shoes with firm, flat soles (preferably not a squishy running shoe). We have all the equipment you'll need at our facility.

Can I try a session before committing?

Absolutely! Here are the easiest ways to get in touch with us to schedule your free session:

1. Schedule directly through our online booking site

2. Call/text Dr. Matt (860-418-7643)

3. Email Dr. Matt (dr.matt@packperformancept.com)
